Provide the appropriate treatment and services to a resident who displays or is diagnosed with mental disorder or psychosocial adjustment difficulty, or who has a history of trauma and/or post-traumatic stress disorder.
Actual harm means the deficiency resulted in a negative outcome that compromised the resident's ability to maintain or reach his or her highest practicable physical, mental, or psychosocial well-being.
CMS uses a scope/severity grid (A through L). Letters A-F indicate no actual harm, G-I indicate actual harm, and J-L indicate immediate jeopardy. Scope (isolated, pattern, widespread) is encoded in the same letter.
